What to Bring to Your First Dental Visit
You can download new patient forms here, fill them out at your convenience, and bring them with you to your first visit. Alternatively, you can arrive early and fill them out in our reception area.
Bring the following to your first dental visit:
- New patient forms
- Driver’s license or identification
- Insurance card or information
- Any copays or money to be collected
- Latest x-rays or diagnoses, if available
